It is the Previews Exclusive Summer 2001 Convention Special and is limited to only 2001 pieces. Jack is resting in a coffin box with a white stake & ribbon closure. The manufacturer was Jun Planning Co., Inc. This is one very rare Jack figure. This has never been removed from packaging.
