This is a vintage silver-tone Miraculous Medal pendant set in an ornate pierced eight-pointed star frame, about 7/8 inch across.
The star frame is what makes this one — most Miraculous Medals are plain ovals, and the engraved fan and quatrefoil panels catch the light from every point.

Markings
No maker's mark found. The obverse carries the full prayer legend "O Mary Conceived Without Sin Pray For Us Who Have Recourse To Thee"; the reverse bears the M surmounted by a cross, twelve stars, and the Sacred and Immaculate Hearts.
